Of course the main issue is that takedown lever was not in the fully unlocked mode when that slide came forward. Now the takedown lever is probably pushing up on the slide, causing it to jam. The take down lever does have a tendency to turn. When I convert my M&P to shoot SIM rounds the take down lever is a pain to get the conversion kit on. Most often it is the striker hanging on the sear. Try not just swinging the take down lever but actually putting pressure on it past the 7 o'clock and holding that while you pull the slide forward and off.
