Infestations spawn under 'overhead mountain' tiles. It doesnt matter if you mined into mountain or just colonized a pre-existing cave. The only ways to prevent infestation are to fill in ever overhead mountain tile or seal up the caves/bunker and run the AC to -17F. You can reduce the odds of infestation with lights, but its no guarantee.

Every incidents in Rimworld is manageable because you could prepare and control how the AI will work. The killbox for raiders work like this so why not for infestation. Just remember infestations spawn in dark places far from open ceilings. So make a place far away from your base that qualifies all conditions.

Alright, so this mod is really simple. All it does changes the Thick Mountain Roofs to Thin Mountain Roofs at game load. You can remove the thin ones but not the thick ones naturally in the game. However infestations are apparently hard coded somewhere in the actual code to only spawn under thick roofs.
